My biggest surprise was that there was more to the rides than just a ride. I can (and do) go through the line for an attraction and exit before getting on just so that I can see the pre-show.

On my first trip to WDW to really see and be surprised by this was in high school. Even though I had been there many times before, I was either too young to grasp this or we stayed off property. Anyway, I was surprised at how peaceful WDW was compared to the craziness of malls, fast food restaurants, and other tourist traps outside it's gates. I actually saw grass, plants, animals on property - go figure!
My trips as a kid were okay at 12 and 16. It was fine when I went with my fiance at 27. It wasn't until I went as a Dad that I fell in love with the place and take the family back every year. The first thing I noticed was the magic and wonder in a toddler's eyes.
I was surprised by the quality of the food as well.

We were surprised it didn't smell like Six Flags (melted ice cream and asphalt).
The first trip to WDW, I was surprised how enormous it was. Since I grew up going to DL, WDW was a shock. At DL you park in the lot and walk into the Main Gate, it is literally right there. So the size was the biggest shock.

The authenticity of everything. Disney doesn't settle for look-alikes. I feel this is so true in Epcot with the recreation of the countries.

I agree Nat, Epcot is pretty fabulous and the King of Morocco story always tickles me.
