When I book through my travel agent she always adds it in for me and I normally do get it for our air. The one time I don't will be the kiss of death........

Yes, I always get travel insurance, too. It is less than $100 for peace of mind, generally. I was on a cruise outside of Stockholm, blew out a knee and had to have medical care on the ship and in a hospital, plus I has significant change fees for my airfare and my Mother's airfare to end our trip early and come home for knee surgery. Costs were hefty and all was returned to me save for the cost of the insurance which was $50.
Often, if parents buy travel insurance, kids are covered for free on parents' policy.

You should also check your work benefits. My family is covered for health related issues when we travel.
Careful with which insurance you take out. Some only cover trip because of medical, not the medical expenses. Some do not cover things like hurricanes, family emergencies, loss of employment.
Many employers insurance policies here in the US do not cover medical expense out of the country like cruises. So make sure you read the policies before you purchase. And I agree, I like Allianz, I've dealt with them on a business level and they were an upstanding client.
I double checked my works' insurance before we left, and luckily our coverage is world wide. Being a multi-national company that makes sense, but as you suggested, your mileage may vary.
